Title: Case Manager II
Department: Foster Care/MR/DD
Summary: The Case Manager II for
Foster Care Services is primarily responsible for providing leadership in the development and support
of the service plans for individuals receiving services through Lutheran Services of Georgia (LSG). As such he/she
serves as an example of an advocate on behalf of these individuals and the caregivers who provide direct care. Reports directly
to the Program Manager.
Qualifications: At least a bachelor’s degree in a social science field such as: Social work,
Sociology, or Psychology and 3-5 years of recent relevant work experience in the human services field which includes direct
casework or supervision of individuals receiving services; Or an equivalent combination of education and experience.
Demonstrated skills in assessment, negotiation, and problem solving. Demonstrated abilities in the areas of being able to: take
initiative, organize, set priorities, schedule, rearrange schedules in order to respond to the most important demands as
they present. Demonstrated skills in writing, speaking, and counseling.
Has demonstrated leadership in a Case Manager position. Must
have a personal multi-passenger vehicle, hold a valid Georgia Driver's license, and have a good driving record and appropriate personal auto insurance.
